Key Information on the School Assessment Form:

Generally, all school assessment forms should clearly show all of the following details:

(1) Student's Full Name

(2) Student Number

(3) Total Tuition Due


The student number on the school assessment form is particularly important as it will be used as the reference number for Bukas's payment to the school.


Using the Form for Previous Balances:

If applying to settle a previous tuition balance, kindly use an updated assessment form or a Statement of Account. This document can typically be obtained by reaching out to the school's cashier or accounting office.
