University of the Visayas Sample School Assessment Forms

The school assessment form is one of the documents you need to upload in your Bukas account to complete your application for a tuition installment plan.

Ensure to upload the entire school assessment form document with all of the following:

• Student's full name
• Student number
• Total tuition due

If you are applying to settle your tuition for the previous balance, kindly use your updated assessment form or a Statement of Account. You will be able to get your Statement of Account from your school.
