What requirements and documents do I need to submit to apply for a Bukas All-Year Plan for PHINMA?

As the Borrower, aside from your personal information, you will need to prepare:

• One (1) school ID or any valid ID (list of valid IDs)
• One (1) proof of residence (list of valid proof of residence)
• Your school assessment form (guidelines)
• Additional Contact Person

For your Co-borrower, aside from the personal and employment information, he/she may need to prepare:

• One (1) school ID or any valid ID (list of valid IDs)
• One (1) proof of residence (list of valid proof of residence)
• One (1) proof of billing
• One (1) proof of income (list of valid proof of income)
